Repeat Prescriptions

pillsWhen requesting a repeat prescription please tick off the prescription on the computer sheet and post it in the surgery in the repeat prescription box.

Requests may be made by sending in an SAE, e-mail lyon.prescriptions@nhs.net,calling into the surgery or following the link on this page to the online form. You may also fax in your request to 01204 462636.

If you are on long-term medication you will be given a computerised prescription re-ordering slip. This allows you to re-order repeat medication without seeing the doctor every time, though you will be requested to attend regular reviews to ensure that all is well with your medication.

Monday to Friday 8.30 - 17.00

For those who find it difficult to get to the surgery we are happy to work with the local pharmacists who operate a collection and delivery service.

We aim to treat all our patients promptly, courteously and in complete confidence. We feel it is important you know who you are speaking to, so our practice staff wear a name badge and identify themselves on the telephone.
