Repeat Prescriptions

When requesting a repeat prescription please tick off the prescription on the computer sheet and post it in the surgery in the repeat prescription box.

Requests may be made by sending in an SAE, e-mail lyon.prescriptions@nhs.net,calling into the surgery or following the link on this page to the online form. You may also fax in your request to 01204 462636.

If you are on long-term medication you will be given a computerised prescription re-ordering slip. This allows you to re-order repeat medication without seeing the doctor every time, though you will be requested to attend regular reviews to ensure that all is well with your medication.

For those who find it difficult to get to the surgery we are happy to work with the local pharmacists who operate a collection and delivery service.

We aim to treat all our patients promptly, courteously and in complete confidence. We feel it is important you know who you are speaking to, so our practice staff wear a name badge and identify themselves on the telephone.

A new way to get repeat prescriptions

Do you get the same medicines regulalry? Do you often use the same pharmacy?

Would you like to get your repeat prescriptions direct from your pharmacy, rather than getting them from the surgery each time?

How does repeat dispensing work?

Your GP will give you a "authorising" repeat dispensing prescription and 6 repeat dispensing issue forms. Take this to the pharmacy and they will do the rest.  Each month you will go straight to the chemist to collect your dispensed prescription.  No ordering at the surgery and no waiting to collect it!

What do I do when my repeat dispensing issue forms run out?

You come back to the surgery, to see the GP and get a further 6months/1 year dispensed.

Complete a simple repeat dispensing agreement?

Sign a repeat dispensing agreement to allow the pharmacist and prescriber to exchange information about your treatment.  All information given by you to your prescriber or pharmacist will be confidential.
